FinalForms is an online form management system where you can fill out and sign important school and sports forms for your child.  Families complete all enrollment forms, back-to-school forms, and permission slips for sports or clubs in FinalForms

Log into  FinalForms 

Click “Update Forms” under Actions

Review and update the information in FinalForms and sign the forms again.

Log in to  FinalForms 

Click “Update Forms” under Actions

Update the address and/or phone number under the appropriate contact and sign the form again.

NOTE:  If you are updating your address, a new proof of residency must also be uploaded.  See “What is Proof of Residency” under What Do I Need To Register a Student”

Go to FinalForms 

Click the black ‘parent login” or Student login” button

Click on “Forgot your email?” or “Forget your password?”

The student’s legal guardian can register the parent.  The legal guardian has the authority to make decisions about the person's health, education, and welfare, ensuring they are properly cared for and their needs are met. Refer to the link Enrollment and Custody Questions for specifics on who can register a student.

Log into FinalForms 

Under Parent, click on “New Account”

If you have a child in Canton City and need to add another child, log into FinalForms under the Parent tab with your email and password.  Click on “Add Student” in the upper right corner.

Student’s Birth Certificate (original or certified copy)

Student’s Social Security Card

Student’s Immunization Record

Proof of Residency in the Legal Guardian’s name. Review this link What Is Proof of Residency for specifics.

Custody Documents, if applicable

Custody paperwork or Divorce decree (court stamped with judge’s signature)

• Grandparent Affidavit/Power of Attorney (signed and filed through the courts)
• Court Journal Entry
• Seeking Custody Affidavit

Legal Guardian Identification (Driver’s Licence or State ID)

Other helpful, but not necessary documents

• Special education documents such as an IEP and ETR, if applicable
• 504 Plan if applicable
• Transcripts

Students will attend the neighborhood school based on your address.  Students are expected to attend their neighborhood school unless they are approved by the district to attend a Choice School.
