Immunization Information

The State of Ohio Immunization Guidelines require that all students provide proof of immunization at the time of registration and as needed per Ohio Revised Code:

Canton City School District has partnered with LifeCare to offer vaccines to eligible students at our district vaccine clinic. The Ohio Department of Health’s  vaccines for Children program provides vaccines free of charge for children through the age of 18 who also are either insured by medicaid/uninsured or American Indian/Alaska Native.

  • Student must be enrolled at CCSD.

    Student must be in grades K-12.

    Anyone aged 19 or older is not eligible.

    Parent/Guardian must be present

    Please bring immunization record, parent ID and medical insurance card.

    Student must be uninsured or receiving Medicaid to qualify.

    Pre-school immunizations are not available at this time.
